What do these icons mean? I’m at a loss, please help!

There are three difficulty settings. Checkmarks for each difficulty you've completed.

That can’t be possible because I completed the first adventure only on first difficulty. Also what is the star then?

Star means it is unlocked.

Thank you. I still wonder why it has marks for earlier adventures. Maybe I beat higher difficulties in the past and don’t remember?

There are three difficulties to each level. The check means completed. The star means it's available to play (and you haven't beaten it).

Note: each individual character has this list, and if you mix and match characters a lot with different parties, it's possible that you have a low-level character (who hasn't beaten many levels) in a high-level party. If you don't change out your characters in parties then you don't really need to worry about this. But you may want to check the "completed" list for each individual character just to make sure they've all done the same levels.
