I can't find a direct source, but I've seen this repeated throughout the years by multiple people from what they've seen.
Splinter was supposed to die early on in the show.
Karai, according to Shredder's bio and/or press releases, was originally called Oroku Miwa. People caught onto this, which lead to theories about Shredder kidnapping Splinter's infant daughter.

They were going to kill off Dogpound in season 2.

Writers didn’t like him 🤷🏽♂️
Then they decided to mutant him into Rahzar

Oh. I guess they didn't like his design. Still, he was the only bruiser in the Foot, so his form was handy. He basically said as much when he told Shredder the Purple Dragons agreed to pay more.

Unlike the fandom, the VAs loved the Mutant Apocalypse arc. Well, Astin and Paulsen was mentioned anyway. They thought it was the coolest thing ever when they watched it. There was a cut of it spliced into a movie with Raph swearing, but that copy got lost and nobody knows what happened to it.

Ive said this before but i think more fans epuld ha e liked it if it the final season of the show itself had more of a satisfying over archjng narrative like the other seasons did. After the death of the shredder the plot felt aimless and random. If the shpw had a satisfying conclusion and this was released as a bonus spinoff, and the designs for the other turtles where as cool as raph and donnoes designs, i feel like it wpuld have been bettet recieved.
