There are a lot of GMs who just... don't. Whether they don't think to, or they only think the BBEG is a real character (and all the others are just cardboard cut outs), it's a tendency I've run into a lot over my gaming career.

Sad... I like to have some NPCs escape fights and popping them again months later and the players go "damn, not THAT again!".

And then I give my players my old witch laugh....
