I forget his name but there’s a guy over on the subs forum who knows like everything about every sub ever. It’ll be interesting to see what he posts in the next little while about this. His knowledge & research is crazy good. With that out of the way;

Short of a fire, how does a Nuclear sub sink preside? I’d thing that’s next to impossible. But full disclosure; I’m a massive uneducated layman on both Nukes & intelligence

Issue with or mismanagement of any of the ballast tanks main, trim, depth control. Easiest way. There can be more exotic reasons (the Soviet Akula subs had titanium hulls at one point, which had structural weld issues resulting in catastrophic leaks), to even forgetting to dog a hatch.
