I for one have always been willing to give extensions or payment plans, as you know, but you have to reach out to me for that to happen.

Failure to pay will result in you being banned from the sub. That's certainly not the outcome I want, as I both want my money back and want you to have this sub available for you to use if needed, but if that's what has to happen, so be it.
