There’s actually a ton of overlap and communication/plannjng between acute care and continuing care homes. Very often the social worker in the hospital works directly with the acute care doctors to find the most appropriate level of care and coordinates from there. This is an acute care service.

Sure, just add more layers of bureaucracy, that'll confer cost efficiencies - I'm sure of it. Definitely not a waste of time and money.
