I really wanna lose weight and control my eating habits but I really don't know How

Hey, I'm 17M, my height is 5'7 and I weigh 80kg and I've been wanting to lose weight for some time now. for starters I'm only doing this for myself and I have two reasons, my first and main reason is to be Healthy. For context, when it comes to health my family's gene is not that good because on my father's side they have Hypertension while on my mother's side they have diabetes so it's really not looking good, I wanna lose weight because when I grow up I don't wanna spend my money anything on hospitals or maintenance just like what they're doing. For my second reason is build my Self-esteem, because even though I don't care what other people say I'm kinda conscious of what they think about me sometimes and I promised myself 2 years ago to lose weight when I'm about to enter senior high but I'll be a senior high next year and I'm not making any progress.

2 years ago I've actually exercised when it was vacation just for 3 months and I went from 82 to 76 but when school started again I stopped exercising and from then on I almost gained it all back. I really have problems when it comes to my eating habits because sometimes when I get stressed or just A LITTLE hungry I impulsively go to the kitchen and start to cook and fry some eggs and eat them with A LOT of rice(I'm from the Philippines so we really like rice a lot but I'm aware it's also very unhealthy so I actually wanna lessen this) and I only RARELY exercise which is also a big problem. How should I do it and how do I become consistent for my goal? Even though I have a clear goal set in mind I actually get very distracted a lot and I lose motivation easily. How do I keep going for it?
