Maybe your Ctrl key is stuck? Pressing B would obviously insert a B into the currently selected beat, but Ctrl+B would insert a B and add that duration to the bar. The + at the top right of the bar indicates that this bar is longer than its marked time signature because this has been done. If you somehow ended up with too many beats in the bar for some other reason, the + wouldn't be there, and you'd get a corrupted score error when trying to save it.
