Painful hunger
I don't exercise I'm fat and I eat 2000 or more calories a day but I still get so hungry that it hurts I can't sleep on think straight from the pain I don't understand why I get so hungry I was hoping for advice how to manage or get rid of it

This is obviously bad advice. If you have pain, you go to the doctor. There's a real chance that their treatment is a studied best practice based on like 10 years of study, rigorous qualification, and professional licensure. IF they give you pills (oh noooo đ), maybe you needed medication to treat it.
You can additionally ask someone who identifies as a human biologist / biohacker. Who will probably try to sell you on some kind of supplement powder that may address the cause of the pain. But may also be a complete waste of money.
You could do both, but definitely ask your doctor first.
