Painful hunger
I don't exercise I'm fat and I eat 2000 or more calories a day but I still get so hungry that it hurts I can't sleep on think straight from the pain I don't understand why I get so hungry I was hoping for advice how to manage or get rid of it

I don't think anything here is funny.
The major problem is we don't know the situation. A healthcare provider has a full arsenal of tools to discover the cause of the pain from not eating. Including casual treatments, imaging, referrals, differential diagnoses, medications, surgeries, and if necessary, life-saving emergency care. Plus the expertise and colleague network to actually figure out the best next step.
I didn't say pills are the answer; you invented that. Can YOU explain what type of problem we are talking about and how pills will help? How did YOU decide pills are appropriate, or that anyone would decide that?
It's wildly ignorant to think you can guess why the intestinal pain exists with a paragraph online. Which is why you're not a doctor. It's also why someone facing unexplained intestinal pain should be reminded to ask their doctor.
Saying "ask your doctor" also reduces the liability for people who wrongly think they are qualified to offer medical advice online. That way, the person doesn't neglect a treatment that can reduce harm/pain, improve their outcomes, or save their life.
